It comes with Headset, cable plugs for Wireless settings, and the MRP Chainguide. (thou, I had to put a spacer to make it fits for the 52mm chainline...)

Shipping was fast, but was initially missing some hardware that was to be included. Customer service was quick to resolve issue with a second shipment. Overall great experience.
I bought this extender as I had to do a 102 mile run (and didn't want to let my friends down), and was totally not prepared as work kept me away from my Creo 2 for weeks.
Install was easy, my display upped my available capacity from 100% to 150%, and I was able to finish my century ride with around 15% of power left. The ability to install an extender, is one of the many reasons why I like my Specialized Creo and Levo SL bikes.
